The same way as for The Rose, and many others biopics. The movie: The Rose was allegedly based on the life of Janis Joplin. Everybody knows Boogie Nights, and The Rose, were based on actual people, because they're too coincidental, but their names were changed for protection.

Events, names and places, are often changed, when films are made, when based on actual people, to protect the filmmakers against legal action, and lawsuits that may occur from the real people who're involved in the story.

For those who struggle with the obvious: filmmakers often use real-life events and people as a springboard for their own fictional stories. They don't necessarily have much interest in a factual depiction of whatever their inspiration is, because it's, you know, art. This is one of those films, so is The Master, so is There Will Be Blood.

What about appearance? Are liberties ever taken with appearance when films are based on actual people?

Is there a source where it says Dirk Diggler represented John Holmes? I certainly never heard it before. To me, the Dirk Diggler character merely represented an embodiment of a younger 1970s male who gets into that industry and lifestyle while facing the consequences of it - same goes for many of the other characters.
