There was the one throwaway line of him telling Michael he was going to be at his side during the prison transfer, but I feel like it would've came across better had there been hints throughout that the doctor was up to no good / contributed to the bus crash / etc.

I thought the "by your side" comment was about as subtle as a sledgehammer. I immediately figured it out once I heard that. Any more hints and I don't think it could be considered a twist.
