I had this happen with my new Ultimate Edition of How The Grinch Stole Christmas.

Go to: https://www.warnerbros.com/customer-service and start a support ticket.

They'll have you snap a picture of your movie case with a post-it with the ticket number on it, and then they should send you a code. Went really smoothly for me, but it is rather annoying when they leave the code slip out.
