If ever in doubt always refer to the 4K spreadsheet. It's very helpful.
As for the code itself, I redeemed mine in MA and it retained 4K rights. I have it in 4K on both Amazon and iTunes. Do believe because it redeemed HD in Vudu it ported as such. While the code might have been a 4K code, MA only recognizes the format during transfer and not the code itself.
In short, yes, you screwed up.
