I agree the first encounter with the medical person was real, but the second encounter did not happen. That whole final day I think did not even happen as she experienced it as she was probably never even in the hospital.

I really enjoyed this movie, but the one quibble I have is that it plays loose with reality vs. hallucination in some inconsistent ways. At some point the internal logic didn't hold and it was clear the movie couldn't continue work if it made the distinctions clear to the audience.
