If anyone could provide information on how to identify any faulty copies just by looking at the packaging/disc then that would be very helpful.

Well, that's handy. I only have a couple of those discs - and my Day was a freebie, so I'm not too freaked out about it. On the other hand, I might have to get Army of Darkness swapped - I bought mine right at release.

Anyone have a link about the issue?

H4 replacement info

If you were affected by a Halloween 4 sync issue and would like a replacement disc sent to you, please follow these instructions:

In order to receive a replacement with the first batch being sent, send an e-mail to ABEHalloween4@Starz.com and include the following:

1) Proof of purchase (photo or scan of receipt or digital receipt from on-line retailer)
2) Your name
3) Mailing address

Your replacement disc will then be shipped to you.

So...this was a Shout release, but I'm not sure if it was exactly their fault, and AB was the company that was doing the replacements. I'm not sure if that is still a valid program. There was an easy way to tell if you had the faulty disc - the correct one had a flaming background on the disc art, while the incorrect one was black and matched all the other discs. (I think, I might have it backwards)

This is correct. If your H4 disc matches the set you have the bad disc.

Also is there a thread like this for Disney or any other company for that matter?

Thanks a lot for this, JCF. Wasn't even aware of the Halloween II mono "stereo" as I usually watch my (Moustapha Akkad Presents) Universal disc. Guess I'll continue to do just that and keep the Scream for the extras.

There's also an 'echo' effect on the audio in the theatrical version of Lifeforce (the Arrow disc doesn't have this issue). (And for the record, the Arrow disc of Comedy of Errors includes the missing frames.)

I wouldn't necessary let the issue be the only reason not to buy the film - the primary, director's cut has no audio issues. Here is a comparison of the two cuts, with spoilers.

The disc for one the films, Fist of Fury, was actually recalled a second time due to it missing the Mandarin mono track.
