Do all Big Bang Theory Blu Rays come with Ultraviolet?

No, many of the seasons were re-released without DVDs or UV codes. This has also happened with several other shows including Person of Interest, The Following, and The Vampire Diaries.

Also, weren't there a couple seasons that never came with the DC?

I have S1, S2, S5, S6, and S7 with DCs, but S3 and S4 never seemed to come with them at all, correct?

Correct.

3 and 4 were the first two seasons released on Blu-ray (in 2010 and 2011), and they were Blu-ray only (no UV/DVD combo packs). Seasons 1 and 2 were the next to be released on Blu-ray (in 2012), and they were the first to be released as combo packs with UV copies and DVDs. Each new season release since then been a UV/DVD combo pack. Some subsequent re-releases of those seasons have ditched the UV copies and DVDs.
