Default Index for Replacements?

Is there a thread that contains an running index of defective discs and replacements? When there is an actual error with the disc, and the manufacturer offered a replacement program.

This would be very helpful as a way for people to check if they have any discs in their collection that they didn't realize were defective, how to identify those discs, and how to get them replaced.

For example, I've heard about a STTNG Season 1 Blu-ray issue, but I bought mine really late in the game, so I'm guessing the copy I got should have been corrected by now.

While the discussion etc. would still take place in the individual product threads, the index would just be added to when a confirmed issue happens and there's a replacement for it. That way, you don't have to try and read hundreds of pages of posts about every product you have to find out if there were any problems with it. You just go to the index and have a quick skim (or search) of the titles.
