Yamaha 465 and direct setting

when the receiver is set to direct what audio is being played. the receiver shows all 6 speakers are on. i have the at%t motorola dvr and have been having audio dropouts for the last year. when i set the receiver to 2ch stereo or direct the dropouts do not happen. this does not happen playing my hd dvd player or my blu-ray player only when i watch the tv. i have the hdmi cable going from the box to the tv and optical cable from the box to the receiver. when it is on direct it sounds like full digital sound from all the speakers
