Yes they always do.. I have yet to see a 4k disc with special features, they always put those on the bluray so yea very unlikly they do a rel without the bluray.

It's unusual, but both things has happened... "Le nozze di Figaro" is only a 4K disc and there was one review on this site (the title escape me and I'm on my phone so it will take too long to find it) that mentioned how good it was that all extras was on both the 1080P and 4K discs.
The norm is still what you wrote, but there are exceptions.
