For those of you that bought Kill Bill from Amazon...

You probably didn't receive your $10 Rebate forms either. I sent an email to
Buena Vista Home Video, and this was their response concerning the forms:

Thank you for inquiring about the Blu-ray Live Action Upgrade rebate offer.

Amazon.com is offering a bundled package of Kill Bill Vol. 1 and Kill Bill Vol. 2 at a discounted price. Blu-ray discs purchased through this offer are not eligible for the Blu-ray Live Action Upgrade rebate. The rebate offer only applies to Blu-ray discs that have specially marked packages (with the "$10 off" upgrade sticker).

Please complete a 3 X 5 card with your name, address, telephone number, and email address. You should send this card along with the proof tab or UPC code for the DVD, the proof tab or UPC code for the Blu-ray disc, and the receipt from Amazon showing when the Blue-ray disc was shipped.

I hope this helps anyone else who probably wants to get their hands on those forms also (or in this case at least the rebates back ).

Haha, yes, the same misspelling! It doesn't make any sense for them to not include it with the Amazon bundle since it's equivalent to Fry's and bestbuy.com's $10 off offer.

Find this a very dumb response considering I've contacted them about forms for Gangs of New York and Signs both which were purchased from Amazon. They didn't give me an Amazon excuse....

I've never been into the idea of cutting my cover art to get these rebates. I think they should have the proof of purchase on a slip inside the cases.

At that price you should donate your rebate forms to someone who needs them on this site (since you probably won't have a valid recipt to qualify
for the rebate)

The Amazon 2-pack is just a copy of Kill Bill Vol.1 & Vol.2 shrink wrapped together. Both copies are Rated R, not NC-17 as Amazon's CS Reps were telling customers who called in.

I was a little confused about this also. I'm just going to contact Disney and tell them I bought it at Best Buy with the Rebate sticker on the outside but that no forms were inside. It is obvious Disney specifically pressed copies without the rebate forms for Amazon stock.
