Disney Reward Code Missing..?? - Blu-ray Forum

I just opened up my copy of Nightmare Before Christmas and the Disney Movie Rewards Code was missing. The slip was in there with a spot for the code, but it was just blank..? Has anyone had this problem and is there any way to get a replacement from Disney? I know it's not that big of a deal, but I like saving up points for my nephew..

yea i have seen that before. Its a PIA to get a replacement code too. You ahve to cut UPC out and mail receipt

you can get the form online for DMR replacement....as jwbbud said, it's PIA.
