These movies are endurance tests, but the pay off is good.
My pick is "Requiem for a Dream." Those who have seen it know how hard it is to watch and yet you can't seem to take your eyes off the screen. This film stays with you long after you have seen it.

What are some of your "Hard to watch but worth it films?"

Alexander Payne's About Schmidt.

120 minutes of crushing emotional despair, followed by the most life-affirming 5 minutes ever. Totally worth it.

Fear and Loathing in Las Vegas, I can almost never watch in one sitting I get massive migraines during this movie lol.

Requiem for a Dream was not to close to my thresh hold. Great movie.

Hostel would be up there i say.

The first time you watch it at least.
