I've been registered with Disney for years, I am in the Disney Movie Rewards and I sent my info in through their site on the Pirates replacement disc.

Other than that, nothing I did to get it.

I went through and it gave a survey of a couple questions (like how many DVDs did you buy in the last year (0), how old I was, how many movies I owned in total (multi-choice))
Then it said I was accepted and confirm my e-mail address and wait for the confirmation e-mail
