My favorite ballad is obviously a Taylor Swift song. lol It has to be Enchanted from her album Speak Now. Great song! The lyrics are brilliant too!

That's one of my faves as well. Limp Bizkit made a decent version of it as well (mashed up with the Verve's "Bittersweet Symphony," which is also a good song).

Otherwise, I'm not sure offhand what my top favorite ballad is. I'm going to have to think about it...
