Blu Ray sleeve template/Amaray Case Template - Blu-ray Forum

Does any one have a Blu-Ray sleeve template so that I can work from.

Ideally I would prefer a Quark Template file.

Where can I get my hands the Blu-Ray logo in either an eps file or a large enough pixel based document?
