Default Godfather Collection Rebate Question

Ok, in an effort to get this question answered, I'm starting a new thread for it instead of posting it in others.

I just got the Godfather on Blu today and am extremely excited. I also own the original DVD Collection (box pictured below). I want to send the $10 rebate in but I can't find any proof of purchase on the original DVD set. For the DVD upgrade rebate, do I need to have the Coppola Restoration or something? Cause that would SUCK.

Here's the set I have:

I've got the same DVD set and I couldn't find it either. I'm thinking the UPC/proof of purchase was probably on a paper insert that was stuck on the back of the box set and was promptly thrown away by myself and many others.

I can't find a phone number for Paramount anywhere. I want to call them and at least try to figure out what's going on.

I've got it with the paper that was stuck on the back. There is a UPC code but nothing that says "Proof of Purchase" like you see on a lot of discs.

My paper was glued so I left it on. Cut out the UPC along with the restoration one and sent it away. I sent mine out a good month ago.

I got this within a week of its release and paid top dollar for it at the time. So you mean to tell me because I didn't see the invention of Blu-ray back in 2001 and threw away the stupid piece of paper, that I won't be able to get my rebate?

I need Paramount's number, I can't find it anywhere online!

If you paid what I paid for it ($34) I'd say yes. Parts 1 and 2 are my two favorite movies of all time, so I'm slightly biased. However, I still can't figure out about this stupid rebate...and I cant figure out how to reach Paramount to call them.

UPDATE: I called random numbers at Paramount and they eventually transferred me to customer service, who essentially had no idea what I was talking about. I'm just going to photocopy the UPC code on my godfather collection (DVD) and stick that on and mail it. I'm not cutting off a piece of the nice box. If they don't give it for me then oh well, I still paid $34 for it.
