Smile Which type of Health Plan should I pick?

Which did you pick?

I am single, mid 20's, almost never go see doctor unless like a injury. My employer (federal) pay parts of medical expense.

Just make sure you cover yourself with a deductible that is not too high. I know you're in your 20's, rarely go to the doctor, etc. etc. Unfortunately, things change.

PPO simply for the need not to get referrals - nothing like doctors recommending their other cronies to go to instead of you having the ultimate decision on who you see for what purpose
