It means it's a hidden audio track or subtitle. Usually the programming of the disc looks at the DVD region of the player to determine whether or not the tracks/subtitles should be visible/hidden from the main menu.

Sometimes if there is a language selection before you get to the main menu, that can also 'activate' these hidden tracks.

Out of curiosity, does that disc prompt you with a language selection?

It can also indicate PiP tracks.

The disc in question is the UK release of The Long Kiss Goodnight. I can't recall it, but I will check it later today.

UPDATE:
I checked it, the disc doesn't promt you with a language selection. You go directly to the main menu, and you cannot choose that hidden Japanese audio track/subtitle, neither from the main menu, nor from the right-click context menu/via the appropriate button of PowerDVD during playing. They managed to hide this Japanese track really well.

Ok. As rdodolak said, it maybe region specific. Have you got a region A machine? If so, try it and see if the JP track appears.
