I've only seen the movie like once, but others in the 4K thead said there's more than one mask in the film:

The back cover is for sure from the film, so I imagine the front cover art is based on photos from the movie as well.

Yes, both the front and rear shots are from the film and the same mask created by KNB, but TPTB demanded the mask be redesigned which was done by Stan Winston and resulted in a majority of the movie being re-shot. However, several shots of the initial mask can be seen throughout. Artistically it would have made more sense to feature the mask the production replaced that's more prominently featured throughout the film.
