Future Shop will have a Underworld: Awakening steelbook exclusively in Canada

It contains a 3D version of the movie and the release date: May 8, 2012

I think it'll be an identical worldwide release like all Sony steelbooks so I doubt a GB will be necessary

Yeah, there's too much lighting on the cover pose. and Selene's expression just looks plain weird.
