Custom Slipcover Template

Hey, guys sorry if this is the wrong place to post this, I found this thread Custom Slip Covers, reading over the 17 pages some people had up'ed some blank templates for slip covers, now only dead link.

Well in a few weeks I will filming my sisters wedding and i'm hoping to put the video on bd-r and make a few copies for friends and stuff, well I would love to make a slip case cover for the video so i'm looking for somekind of template for can be printed on A4 paper and cut, would anyone here know where I can find this kind of template, thanks you.
