Depends, to be honest.

Rambo had its gutty parts, however the Saw series and those torture movies really tend to show the most gutts. So them, is my vote for the most gutty.

Goriest movie would have to be Dead Alive (Braindead), but the goriest movie that actually made me sick somewhat was either The Beyond or City of The Living Dead. I mean
[Show spoiler]spiders eating out a person's tongue and eyes or someone vomiting real entrails.

Stop you are making me hungry. Thinking back to when I was younger I would have to say the original Dawn of the dead grossed me out the most. Todays films are far worst but I have had many years to assimilate.

Some good ones mentioned here but I'll put my vote in for Riki-Oh: The Story of Ricky. One need not ask why a prison would need a meat grinder big enough to grind a cow whole.

I just watched the trailer on imdb.com... you can actually see the grinder in the trailer! Now that brings back memories, I may just have to go downstairs and a have a look. I haven't watched that movie in ages, it was one of the few that survived the great kung-fu movie purge of 2008.

Hellraiser II: Hellbound, when the mental patient starts slicing himself with the cut-throat razor because he thinks that maggots are crawling all over his skin. Abso-lutely repulsive!

Having said that, the first four Hellraiser films know their target audience and aim for at least a couple of Gross-outs through-out the series. Plently of gore in films 3 and 4, but that scene in 2 was too much...
