Enterprise is maybe my choice for the worst ever. All of the other Treks have have pretty great opening title themes, and here comes Enterprise with Christian Soft Rock. I liked the show, though. Especially when they started doing the mini-arcs.

Also ones that really irritate me:

30 Rock
Scrubs
The King of Queens (seasons 2-9)
Arrested Development
Clerks: The Animated Series
How I Met Your Mother

All of these have me holding the remote for the first few minutes, ready to pounce on a button to end the pain when the opening pops up. They irritate me way, way more than they should. I hate them.

I like the AD opening theme its simple and not overdone.

In fact, sounds like they originally wanted to use the pompous-fanfare ending theme, but that would've made the opening credits feel too much like "Voyager" (a bad, bad show with a good theme and opening), and at that time in the franchise, NO fan wanted to invoke the dreaded V-word.
But faux-Bryan Adams could hardly be called a step up.
(And actually, Scott Bakula is probably the third best captain the Trek franchise ever had, but oh Harriet, that third season... )

In fact, can't think of many classic reruns with bad themes, that's a toughie--
I was going to say "UFO", but even that one's got its own particular 60's cool, albeit doesn't compare to the two Space:1999's.

Two and a Half Men...craptastic opening theme, for a show that was pretty watchable at its best. Just hearing it from another room makes me want to yank my eardrums out with a rusty set of pliers.
