Some Steam codes are region-specific. If you can't redeem it, maybe it's because it's not for your region, not because it was already redeemed. When you post these codes, better to specify what region(s) they are for.

I think it is because someone tried to use it but it didn't work for them, due to the wrong region. I could be wrong.
