All solid picks so far. The only one of the above lot that I never found disturbing in the least bit is A Clockwork Orange. While it contains subject matter that could certainly be deemed disturbing, its playfully satirical/tongue in cheek tone stops it from actually getting under my skin. Similar to the even more zany A Serbian Film.

Pink Flamingos is also hilarious, but the actual animal abuse and final images push it well into disgusting territory

I'm shocked I think I was the only one who put White Rabbit (2013) on my list. I can't imagine anyone who wouldn't find it disturbing unless its just so obscure not many have heard of it.

I think Irreversible is a great film & one of my faves but not as bad as people make it out to be. I find the fire extinguisher scene more disturbing than the other scene

Just watched Martyrs the other day, didn't find it that disturbing. Now if they actually showed
[Show spoiler]children being raped, then that would be a different story
. Then again i don't find most films disturbing which is part of the reason i didn't make a list, that and i haven't seen most of the top ones that are considered disturbing.

That other scene is the most disturbing thing I've ever seen in a movie.

Now that we've seen that "Irreversible" didn't quite make it into the 'top 5', but rather was actually voted into the sixth place position, I must wholeheartedly implore you NOT to kill yourself, as suicide is generally considered to be rude, naughty and bad for your complexion...

Plus, I'm absolutely certain that they'll find a way to pin it on me somehow, and I'm waaaaay to tired to 'take it on the lamb', as it were...

I figured for sure Serbian would be top 5, just based on how well-known it is. I guess that leaves Salo as a shoo-in for the top 5, and probably Slaughtered Vomit Dolls as well (as it should be).
