Default Audio video sync problem w/insignia

My parents got a new 42" insignia LCD at bb last Friday got it home and there are a couple of problems. I haven't had hands on with a new LCD before this so I am seeing what you guys have to say. First problem is the audio syncing with the receiver. When we turn on the dcm the audio is way off. I got a optic cable running from the blu Ray to the receiver, and a hdmi running to the tv. Is this what is causing it? The other thig is my dad was watching golf and when someone hit the ball it looks like there are three balls. I can tell my folks are not pleased at all with this tv. Any ideas.

I just bought the 55" Insignia, NS-L55X-10A, and I had the Audio delay problem as well, using fiber to my receiver. I called Insignia tech support and they suggested I swap out the STB's and the 360's HDMI connections. Apparently she wanted me to do this while the TV was turned off and I didn't turn it off, so I turned it off and then back on and the sound was working fine. It could have been either the swapping of the HDMI connections, the powering off/on, or both, but my audio is perfectly lined up now.

I can't speak to the video problem though, our video is working great.
