quick Rock Band question...

Guys I'm having a real hard time making the transition from medium to hard on guitar. It's not so much the speed at which the notes come at me as it is the addition of the 5th fret (orange). I just cannot seem to make my fingers react correctly. My question to you guys who play, how do you deal with this? Do you stretch your pinky out to push the orange button or do you slide your whole hand down where the third finger is on blu and the pinky is on orange? I'm just having a complete mental block on how master the addition of the 5th fret!
