I know I am really late in responding to this, but our generation grew up on Doom as a teenager, that had plenty of blood and guts being thrown around. We turned out very well. I think most of the Mature ratings are overblown myself.

If you think Fallout 3 might not be the right game for the kid, then Uncharted: Drake's Fortune would probably be an excellent alternate choice for an adventure game. If he likes platformers, Little Big Planet is superb.

I think thats the best response so far ... my op its a great game but you will get everything he mentioned above good luck.

I agree with that as I grew up on duke nukem, doom/ doom 2. I just think even those titles would be more fun for a kid then; conserving ammo, making decisions about how to save things/ where to go, tons of walking.. while I enjoy it now I dont think i'd have played it to much being a kid.

i got fall out for all 3 of my boys ranging in age from 9-13. Games like this are ok. gta and saints row are to much though.. draw the line there

No because I'm playing midnight club la but I did see about 60% of the game walkthrough on youtube. It looks like an fantasy sci-fi action movie type of content.

The boody gore slasher & monster ghost story in horror stories seem more of an lasting & even detrimental effect since they tend to play on peoples fear of the unknown.

I think that its okay as long as you think your kids are mature enough to handle it. Its just like letting you kids see 'R' rated movies, its important that you are there to supervise and answer any questions they may have.
