You know, at first I thought cover B was great, but now I actually like cover A a lot more. I like its minimalist approach and the heavy sense of dread it conveys.

I like both, but I definitely prefer A.

As someone who remembers when the first one came out I obviously voted B

B is the most metal cover art I've seen in videogames, and is the obvious choice for me. Nothing wrong with A, I like it quite a bit more than the generic cover art, but B is quintessential Doom to me.
