I bought the Czech version from superstarshop.cz and just swapped the artwork with the older UK release that I owned. You can also get it from Hungarian, Russian, Polish, Hong Kong and Mexican e-tailers.

The extended cut never made it Blu-ray.

I realise that this post is over 12 years old but just in case anyone is reading back through this thread, it is worth flagging that Sony subsequently replaced the headbutt in the original cut too, so while the original UK Blu-ray is uncut, the re-released version is not.
