This site and a couple others mis-print the aspect ratio of this blu-ray. They say it's 2.35:1 when actually its 1.85:1. I found a site online that posted the back cover of the box art. Unfortunaly I didnt bookmark it.

Yeah it's not a full 2.35:1 movie.

Great film and transfer though I doubt I'll be visiting this again anytime soon. Maybe once in a year or two is enough.

The review on this site lists the feature-length "Fear of the Flesh" doc with the extras, but the version I'm seeing on shelves doesn't mention it. Are there multiple versions, or is this just a packaging oversight?

It's a packaging oversight. I held off buying this film for years because I didn't think it came with the "Fear of the Flesh" documentary but I decided to finally pick it up today. I checked the special features menu and it is included. I haven't watched it yet but it was refreshing to see there. I'm not sure why they wouldn't include this on the packaging. When I picked up Jacob's Ladder years ago on DVD it didn't list any special features but it comes with a bunch of them so I guess from time to time distributors overlook this things.
