Studios typically cut down on offensive content when the MPAA gives a film a high rating (like NC-17). However, films are not required to be rated at all, but it could hurt box office sales if they aren't. Some theaters don't show unrated films do to potential box office failures, but more popular films have been shown in the past without being rated.
