"entertainment" with boy or girl was common pratice back then, both in Roman/Greek cultures.

Incest was frown upon in Roman times and considered as nefas (against the laws of gods and man) I'm not sure if Commodus wanted to have sex with his sister Lucilla, although Commodus is said to have incest with other sisters.

Lucilla was implicated in plots with members of the senate to kill her brother. In 182, following an assassination attempt, Lucilla was exiled to Capri and later executed on Commodus's orders. Later Commodus was assassination by a Roman athlete, Narcissus.

The character of Maximus is fictional.
