Yup. I received the same and sent them the info in that format. Because I bought the steelbooks, I included the 4 titles together and carried that combo throughout their template. I sent this back on Sunday and have yet to get a response.

I e-mailed them letting them know I no longer had a receipt (as I purchased from a local store and don't keep my paper receipts) and they responded with the form letter, I'll let others know if they will allow folks like us access to the replacement discs or not. My hunch is I may be ****ed, but I guess that's what I get for shopping local lol.
