Yup. I received the same and sent them the info in that format. Because I bought the steelbooks, I included the 4 titles together and carried that combo throughout their template. I sent this back on Sunday and have yet to get a response.

The email I received today was mostly the same, except they didn't request the UPC code (though it's shown in the pic of the back of the case).

I e-mailed them letting them know I no longer had a receipt (as I purchased from a local store and don't keep my paper receipts) and they responded with the form letter, I'll let others know if they will allow folks like us access to the replacement discs or not. My hunch is I may be ****ed, but I guess that's what I get for shopping local lol.

I email them at 7:12PM on Saturday just asking about a replacement program and got a reply a minute later with the same process descriptions others quoted here. I assumed it's an automatic reply.

I sent the request info 20 minutes later and haven't heard back, but with other replacement programs (not WB) I also usually didn't hear back and just got the corrected discs eventually.
