Disney Reward Code Missing..??

PVJAG Power Member Member since:Jan 2008 Location:NYC Trading Score:100	Disney Reward Code Missing..?? I just opened up my copy of Nightmare Before Christmas and the Disney Movie Rewards Code was missing. The slip was in there with a spot for the code, but it was just blank..? Has anyone had this problem and is there any way to get a replacement from Disney? I know it's not that big of a deal, but I like saving up points for my nephew..
