Lucas has said all of this... but that doesn't make it true. He's not being honest with regards to how this developed.

I mentioned this in another thread (I think?) but people really should pick up a copy of The Secret History of Star Wars. During initial drafts of The Empire Strikes Back, Anakin Skywalker and Darth Vader were still separate characters and the plan for SW movies was to do about 12 (later reduced to 9) of them that were only vaguely related to each other.

The basic outline of the "entire saga" changed radically throughout the years and didn't center on Vader until at least after ESB, even not beyond. Again, Vader was generic bad-guy until then 2nd or 3rd draft of ESB made him Luke's daddy. (Not even Leia's at that point.)

Lucas likes to now portray this as some sort of "grand plan" that he cooked up in 1972 or whatever. That's simply isn't true.

We'll leave the meaning of the title as one of those certain point of view things.

I must admit that I do take anything GL says with a grain of salt.

Either way, going back to where this started (the Hayden/Sebastian ghost issue), it doesn't change the fact that Anakin did not die a Sith. For me personally, I couldn't care less whether the spirit is that of a young or old Anakin. Like I said previously, I thought the whole scene was a bit much. To me it feels more like watching a play... When the whole cast comes out for a bow at the end. It was just kinda goofy to me. Anyway, there's a case to be made for either the Hayden or Sebastian spirit to show up. All I'm saying is the argument that Anakin was still a Sith at his death is not a valid one.

While I agree with that, I do happen to remember that way back around 1980, one of my friends said that he either saw or read an interview with Lucas, that he stated that Vader's costume and disability was the result of his last confrontation with Obi-wan on the edge of a volcano.

So that part at least was not recently made up, but goes way back.
