It's three swords, since, you know, it's the third season, but other than that, it's a reference to the title of the novel; a Storm of Swords.

The first part of the fourth book actually takes place during the last part of the third book. I expect season 4 to have the rest of book 3 and some of both 4 and 5. Then season 5 will include book 4, 5 and the first 200 pages of book 6 (we know something important happens there that was moved out of book 5, but that the tv series might want to include). Then season 6 and 7 will have the remainder of the books.
Yes, book 5 is enormous, but can easily be condensed since the important plotlines aren't that complicated. The showrunners will probably be smart enough to move some of 4 and 5 to an earlier and a later season, but they would do well to not spread it out to two seasons. Seven seasons is a luxury most series don't even get, so counting on eight might be a bit too optimistic. And they will need those last two to finish the story. Who knows how big the last book is going to be.
