It's most likely a combination of all. It means you're gonna have to experiment a little more. I highly doubt the banana plugs are the cause.

I personally don't give sound levels much thought. As long as your AVR/AMP-PreAmp combo reproduces accurate sound, who cares what levels they are. If your AVR/Amp is straining at any level, then look into getting a more powerful unit. If not, a number is just a number...
