For Martyrs, the US R-rated cut is an edited version. The original cut is the one that's labeled Unrated and would be an NC-17.

But, it is still rated R, right?

If anything, that just reaffirms the fact that it is a hard R.

The OP said nothing about it having to be the theatrical version or original cut.

It's ironic how the original X-rated versions of 80s and 90s action movies would probably be considered less violent now than most of today's 'hard Rs'. The MPAA's standards have certainly changed.

True. They can still all agree on the evilness of male nudity, though.

From about 20-plus years ago, I can think of a few: Natural Born Killers, Angel Heart, and Basic Instinct.

Sabotage (with Arnold) is rather nasty.

Ones that come to my mind are:

Rambo (2008)
Evil Dead (2013)
Halloween II (2009)
The Devil's Recjects (2005)
I feel you. So much of what I miss of the old movies is not present now. The Terminator is a fine franchise that steadily got softer and more cleaned up. Partly due to filming techniques and now cg. I miss the dirt, trash, smoking, graffiti etc of the 70's/80's. A few that come to mind:

Videodrome
Robocop even the R cut was rough
American Psycho
Bad Lieutenant again even r rated was harsh
Romper Stomper
American History X I don't believe would be possible in 2016
8mm content
Wolf of Wall Street was a pleasant surprise of goodness
Cruising I don't believe could be made today
Death Sentence
Death Wish series
Full metal Jacket I don't think would get a pass with Gomers treatment

Some of the British football hooligan films are quite brutal for their melee combat and language. I don't remember the titles but 15 was the rating I believe
Not Movie, but Sons of Anarchy was pretty hard hitting for cable TV. Some of it was far more brutal than many R rated films

Texas Chainsaw Massacre: The Beginning

Matt Bomer's torture was pretty gross.

Alright then, here are the hardest movies I know of from the past decade (R-rated, unrated, NC-17, etc):

Antichrist (sex, violence, brutality, tone, content)
Apocalypto (brutality, graphicness)
Berserk films 1 - 3 (blood, gore, sex, rape, demonpocalypse)
Blue is the Warmest Color (sex)
The Butterfly Effect (tone, content, violence, brutality)
Children of Men (brutality, tone, themes)
City of Life and Death (brutality, tone, content)
Deadpool (blood, gore, language, sex)
District 9 (blood, gore, language, content, themes)
Downfall (violence, brutality, tone, themes)
Django Unchained (blood, language, themes)
Eden Lake (violence, blood, content, themes)
Elysium (blood, gore, language)
Funny Games (violence, brutality, tone)
Girl with the Dragon Tattoo (either version) (tone, content, sex, violence)
Gone Girl (tone, content, sex, violence)
The Hateful Eight (blood, language, themes)
High Tension (violence, content, tone)
Hostel 1 - 2 (blood, gore, brutality, content)
Hot Fuzz (blood, gore, language)
The Human Centipede (content)
The Human Centipede II (content, blood, gore, sex, rape, tone)
I Saw the Devil (blood, gore, brutality)
I Spit On Your Grave (rape, blood, gore, themes, content, tone)
Inside (blood, gore, content, tone)
The Passion of the Christ (brutality, graphicness, tone, content)
Kick-Ass (violence, blood, language, brutality)
Kill Bill Volume 1 (blood, gore, content)
Kingsmen: The Secret Service (blood, violence, language, content)
Martyrs (blood, gore, content, brutality, themes)
Moebius (tone, content)
Mother (tone, content)
Nymphomaniac (sex, violence, content, themes)
OldBoy (either version) (brutality, tone, content, sex, blood, gore)
Planet Terror (blood, gore, content)
Prisoners (tone, themes, brutality)
Poultrygeist: Night of the Chicken Dead (blood, gore, sex, content)
Rambo (blood, gore, brutality, content, themes)
[REC] 1 - 2 (blood, violence, tone)
The Revenant (violence, blood, rape)
Saw (all of them) (blood, gore, tone)
A Serbian Film (sex, blood, gore, content, themes, tone)
Shame (content, sex)
Sicario (tone, themes, violence, blood, sex)
Snowpiercer (tone, themes, content, violence)
300 (both movies) (blood, gore, sex, nudity, brutality, tone)
Tokyo Gore Police (blood, gore, content)
Tropic Thunder (violence, gore, language, drugs)
Tucker and Dale vs Evil (blood, gore)
Turbo Kid (blood, gore)
12 Years a Slave (content, tone, brutality)
Valhalla Rising (blood, gore)
Wanted (blood, violence, language)
Watchmen (nudity, language, brutality, blood, themes, content)
Wolf of Wall Street (language, drugs, debauchery)
Zombieland (blood, gore, nudity)

Of these, I consider Tokyo Gore Police the bloodiest movie I've ever seen, Human Centipede II the most disgusting, and A Serbian Film the most disturbing.

However, none of these are R-rated, and I don't really know what the hardest R-movie I've seen actually would be.

This is a great one, because I skimmed audience reviews out of curiosity sometime earlier this year, and pretty all of the negative ones were calling it porn. I can't imagine what minutiae the MPAA wanted Scorsese to edit in order to attain that rating.

I know I've seen more (like the Final Destination series and such), but I figure I got the essentials in there.

Alright, here's a few that actually received a hard R Rating. No Unrated/NC-17 rated stuff here:

*Exorcist: The Beginning (A lot of gore and violence was there because Paul Schrader's version did not deliver what Morgan Creek wanted..though it did deliver what to expect from an Exorcist movie. The Renny Harlin cut is really silly trash that is better than The Heretic, but still really silly).

*Evil Dead 2013 (So graphic, it had a few frames cut out to get an R-Rating)

*Super (For a good reason. I would argue this earned an even harder R than either Kick-Ass)

*Rob Zombie's Halloween 1 & 2 (A lot of the subject matter really earned the R-Rating for both films, violence aside)

*Texas Chainsaw 3D (Though it received an uncut release overseas, the gory bits in the R-Rated cut did earn a Hard R)

*My Bloody Valentine 3D (Gratuitous nudty? Check. Silly yet gory violence? Check.)

*Drive Angry (Same reasons as MBV 3D earned its R Rating, though the violence was a little more surprising in this film's case for the uninitiated)

*The Town That Dreaded Sundown 2014 (The violence definitely earns the film a Hard R, but for money its that near pornographic sex scene with that cute blonde chick that really does it).
