Default loose discs

Has anyone ever received an order from amazon with loose discs?

I've only had it happen once on a single disc DVD and a couple times on a multiple disc DVD box set both from amazon.

The single disc DVD was badly scratched but it still played fine. But will the scratches make the disc rot in the future?

From another vendor, B&N IIRC, I once got a broken BD case. Not just chipped, but totally unusable. It was for Firefly and since no one sells those multi-disc cases (or do they?), I am forced to house the discs in two separate cases

Amazon has packed things well except just one time. On one order, I cancelled an item so I think they decided to use the bigger box they already had prepared or something. There was no extra padding. You could shake the box and use it as a percussion instrument with the BD bouncing around in it. Fortunately the BD was fine.
