Unhappy Continuing Lip-Sync issues

I continue to have lip-sync issues, not just between my Blu-Ray player and HD cable, or between one Blu-Ray disc and another, but from scene to scene within the same film. The problem is MOST common with Blu-Ray discs, but I've also had it with SD discs of old B&W movies from the 1930s.

I have a Pioneer Elite BDP-09FD set for "Source Direct" with the lip-sync delay set to Manual, 0 ms. I'm using an HDMI connection to feed an Anthem Statement D2V, with the delay set to 24 ms, as recommended by Nick at Anthem's tech support. From there I use HDMI into a Pioneer Elite Pro 150FD.

Lip sync wandered all over the place the other night when we watched the Blu-Ray version of South Pacific (Dolby TrueHD 5.1).

Any thoughts?
